Dear employees,

All current benefits will terminate on December 31, 2024, with the exception of the Disability plans. 

If you are currently declining healthcare coverage, your opt-out election will terminate December 31, 2024. If you wish to decline healthcare coverage for the 2025 plan year, you must enroll and elect this option. Additionally, you will need to submit the Declination of Healthcare Coverage Affidavit with proof of current other group or stated-funded healthcare coverage to FBMC by the enrollment deadline via eFax to 866-879-8689.

If you do not enroll during this enrollment period, you will automatically be assigned to the Cigna SureFit (employee only) Network healthcare plan. This plan requires the selection of a Primary Care Physician (PCP); therefore, Cigna will assign you a PCP based on your zip code.

NOTE: In accordance with Internal Revenue Code 125, NO changes will be accepted after December 16, 2024, unless you experience a qualifying Change In Status event.
